How to be a good "mentor" to someone with a new sci?

So mentor doesn't feel like the right word, but you know what I mean, someone who has been injured for a while, then you meet/know someone who is newly injured. I definitely had someone to mentor me through the fucked up part of being a new SCI and navigating through rehab and reentering "real life". She has since passed away, and I actually never met her in real life, but she was an incredibly important person in my life nonetheless.

Recently someone I know somewhat peripherally was injured and I reached out, but wasn't sure what to say. Obviously I know there are times that someone newly injured would not want to converse with a chronic SCI like myself, because obviously the hope is that the new injury will not be permanent or at least there will be significant recovery. So I sent a simple email like "hey, sorry, sucks to break your back. Been there. Lemme know if u want to talk. If not that's cool too. Buena suerte."

Obviously couched it a little more diplomatically than that, but after rewriting the email a half dozen times, I was never able to make it "feel" right. I guess the important pay is to let them know you're there to listen and advise if they want, but what would youse guys say in that situation?

schedule? muscle groups, how many times per week for progress?

I'm trying to improve so I exercise regularly. But I can't do everything everyday so I'm trying to figure out a schedule that will help. I do exercise from my chair, then I also use a up n free walker which I was doing almost everyday. Now I decided I need to try the regular walker with platform attachments. And I try to use a easy stand glider. I can do my exercise from my chair quite a few days a week on my own time with no assistance so not too much of a problem there. But with two walkers and a glider I need assistance and it's usually afternoons after a long day at work. So I can only do one of those things a day. Up n free is good, I just need help getting in it and I can walk unassisted, but I realized the seat is not allowing me to progress like I should if I want to improve and see better more natural ambulation. So my wife started helping me with the regular walker, not easy, I can take about 5 steps so far. So I think I need to keep trying but also other days use the up n free so I can go 150 steps at least and keep my endurance. And then I have the easystand glider that I know is good for me to use. And then I have other training that I know would be good. How do I portion out what I do without not doing something too little to be of much value?

A friend told me today that he thinks if you work a muscle group really well two times a week that it should be enough for success. Thoughts? I'm not trying to do the minimum for success but trying to figure out to do several different things enough to benefit.

A regular walker that I need help with but improve the more I try, a gait trainer that lets me go farther but limits some progress overall, a easystand glider that I'm not actually walking in but am on my feet - As far as the importance of each of these how would you rate them?

Muscle groups like arms, legs, core, how many times a week if one expects to see progress?

Weekends I should be able to do more but during the week am looking to make the most of how and what I train.

I appreciate any help with scheduling workout ideas.

Best Hand Controls for Mazda 5 - recommendations Please?

Folks, I have a 2014 Mazda 5 and would like to know which hand controls you all use on yours? I have my MPDs from my old Mazda MPV but they look Way too bulky for the "5". I would like to find some which provide the cleanest, least obtrusive installation as I still have to get my legs in there. So, please tell me what you use? New T7 injury

Hello all,

I am new to this forum. My husband is 32 and suffered what we believe is a T7 complete injury in December. He went in for abdominal aortic aneurysm surgery and, we believe, suffered a spinal cord infarction. He was in ICU for 52 days, a step down unit for 10 days, and now he is on day 4 in inpatient rehab. He has had medical issues his entire life, but this is a whole different ball game. We are at the point where we don't know what we don't know. We know that the following things have to happen:
1. He has to get a chair. His inpatient rehab will be evaluating him and helping with this.
2. We have to make modifications to our home.
3. We have to figure out what resources are available to him for ongoing care after inpatient rehab.

We really wanted him to go to Shepherd Center, but they wouldn't take him because he is on dialysis due to an acute kidney injury, also suffered during the surgery. So he is at rehab at the same hospital where the surgery took place. Maybe he will be able to go to Shepherd at some point in the future. I know we have a ton to learn--I think this forum will be very helpful. Thanks.

Preparing to renovate for accessibility--platform lift recommendations?

Hello!

This is my first post. My husband suffered a T7 injury in December as a side effect of aortic aneurysm surgery.

We own a 2-story 1926 bungalow in Tampa. We love this house and selling is not an option for us (we would likely take a bath on it, since we just bought it a year and a half ago, and we would still be in need of accessible housing but we wouldn't have any money to spend on making it accessible). So we are interviewing contractors to start renovating.

We know we are going to need a platform lift to travel up and down the stairs. (He has been in inpatient rehab for 3 days so it's way too soon to know whether he'll be able to transfer... so I would rather plan to install the platform lift.) We want something that will require a minimal amount of reinforcement in the wall. I know this will be approximately a $10,000 purchase, and it's something I know almost nothing about (and I'm not in the habit of spending $10,000 on anything without researching it thoroughly). Does anyone have a platform lift they can recommend? Are there any that we should steer clear of? Advice would be greatly appreciated. Thanks!

Power chair and Medicare/Medicaid

I'm finally having to get a power chair to help me out some since i have this ulnar nerve entrapment in my left elbow. My Tilite Aero-Z is still going strong and i will be using it a lot.

Has anybody on here with Medicare/Medicaid gotten a power chair lately? I'm wanting to know what brands and models they will pay for these days.

I'm used to picking and specking out my own chairs. I've been doing it 14 years. I don't want to just take whatever the DME wants to give me.

Any ideas on good chairs with tilt would be greatly appreciated. I don't know anything in the power chair game.

Shin Splints

Has anyone ever had to deal with this before?

I was running on the treadmill today and my shins got sore. I didn't think anything of it as I just assumed it was regular fatigue. However, several hours later and they are still sore. I've been trying to find out what's wrong with them and so far, everything is pointing to shin splints.

For now, all I've done is RICE it. There's no swelling but I'll see how it goes in the morning.

What are your thoughts?

Any older sci patients out there dealing with partial rotator cuff tear

I have had a partial rotator cuff tear for about 6 months now and was wondering if anyone else has advice on managing the injury. Cotizone shots are out for me as I had a bad reaction when I had one for my injured thumb a few years ago, it sent me weird in the head for some reason and my doctor said I reacted to the steroid component which made me very moody almost aggressive . I am a incomplete T4 38 years post transverse myelitis with chronic neuro pain to add to my joys. I saw a physio once every 2 weeks for treatment and do my rehab exercises at home now as acupuncture didn't do much good

Does anyone get their chair weighed? If so, what is the weight of your current manual use chair? I had my chair weighed recently and it came in at 32.6 lbs, which seems way to heavy feeling. Now i'm regretting letting my grievances with Tilite cloud my better judgment and put me back in a Quickie.

Swollen leg red and warm with UTI side effect? have others had this?

Just met with Infectious /disease Doc about chronic UTI Over past 11 years have 50 plus UTI's various bugs cultured at over 100,000 Of those 50 or so infections about 12 have been serious 7 day fever very sick etc. 2 of those episodes including the first severe one in 2004 % day hospital stay and the most recent urine culture was negative on almost all the severe episodes my left leg swelled up got red and warm to touch I thought this was a side effect . My Id doc says no possible side effect of uti is swollen leg most likely reoccurring celluitus. Wheras i agree on the negative culture episodes confused on when i had >100,000 colonies Curious if others have had UTI and celluitus symptoms at same time?

Ford Transit Reviews???

Does anyone have the Ford Transit (the larger van, not the Transit Connect) as a wheelchair van? I'm looking at this van, but it sounds like a lot of adapting. I'm just needing the lift, hand controls and rotating drivers seat to transfer into. We also need seating for about 9. Not asking for much.

How come the dentist won't remove an impacted tooth, after a knee replacement was done over 6 months ago?
This relative is also in her upper 80's for age.

what muscles / movements in the core are good for stabilizing?

Last night I sat on the edge of my bed with feet on the floor and holding my wifes hands I leaned back and practiced sitting up. I really felt my abs working I think. Today I reclined my chair and held two handles attached to the wall in front of me to help me do assisted crunches. It felt like a really good exercise. But am wondering should I also do diagonal situps to work different muscles? What do practicing situps help with in realtion to functional movement? When I move my arms my trunk may fall over due to lack of strength therefore limiting some of my arm function. What muscles would help core stability if worked and what are some good exercises for them? Would the assisted situps be an important thing to practice? Thank you

Tipping Point - No Treatment for you!

I'll be 44 at the end of the year and paralyzed 31yrs. The years have been about survival and very rough on my body.

It's a shame I won't benefit from any of the potential treatments we read about now. Many of us ole timers won't.

What a shame.

I've got wounds on my feet, backside, on/off along the bottom bones of my leg .. yet circulation is excellent .. my stamina is non-existent. Stopping work probably saved my life but the quality of it now .. jeezus. I'm waiting for the end.

I'm just in awe that my whole life was this ... I can remember reading on this site from 40-50yr olds saying the same (that treatment would never reach them) and feeling badly and encouraging them to think positive.

Now here I am. Nothing coming out this year or next. Not even in trials. Ain't life grand?

Drop Foot!!

Since I left rehab, I've kept up with private physiotherapy at first I was going five days a week for quite some time then four, then three, then two! But I fell off the train so to speak for about five months, got bedsores so I couldn't wear my braces needless to say my drop foot got worse! Now i'm back at physio, My therapist thinks there's still movement she says is definitely tight and shortened but she thinks we can do some good still. Now I get in the standing frame and all my weight is held near my toes my heel won't touch yet it's gotten better over the past weeks slightly especially when I'm in the standing position and she tries to loosen the gastroc muscles with manual work. But does this sound like it can be reversed, or is this likely a contractor and permanent?? I also have very expensive leg braces for night that I can actually increase the amount of weight they push against my foot and different angles and adjustments

T6 a I and just had hernia surgery, now I can't void my bladder like I did before

I am a t6 incomplete para and have used external catheters for about 30 years. I just had hernia surgery last Friday and I have not been able to void my bladder without using an intermittent catheter since the surgery. Is there anything I can do to get my bladder working for me again.

Accessible vacation ideas for a family

My family consists of my husband (t-2 complete para). Me (no disability, but several health issues lately). Our two kids aged 9 and 10. I need to come up with some good vacation ideas, I'm running out.

We have been on a cruise. The family loved it. I got seriously sea sick the entire time. But unless someone has a sure fire way to avoid sea sickness, I am never doing it again.

We have been to Disney World a few times and its been a lot of fun and very w/c friendly. But we don't want to always go back there.

We have rented accessible condos and houses and just enjoyed a destination. Thats fun, sometimes. But the kids are getting a bit bored with that now that they are older and want more action.

My husband isn't in a place where he can be sporty, so a lot of those resorts look like "sorry kids, can't do that" would be all we would say. So I don't know.

Any good, accessible family vacation ideas?

are parallel bars best route for practicing safely?

I use a up n free walker, it has a bicycle seat and strap for safety. I just tried a regular walker for the first time in a long time, trying out the platform arm attachments for better posture and support. The idea is to be actually fully on my feet, it was scary compared to a seat and strap. I took one and a half steps. I think if I keep at it I will get better, but I'm discouraged. Anyway, for being on your feet and practicing walking, would parallel bars be the best route to begin with? And any ideas for low priced solutions? Thank you

New pain

Boy, I really got the short end of the stick with all of this "disability" stuff. First, when I was 15 I cracked my skull. It was a miserable month after that but I survived with very little wrong with me afterwards. I was still able bodied and still didn't have to take any medication for it. When I was 22 I broke my neck hitting my head in the bottom of the pool. I'm pretty sure that left at the pretty bad concussion but we weren't too concerned about that considering I couldn't move. I've had a few concussions since and wham, since 2010 I've had epilepsy. Seizures drive me nuts and the medication is even worse. I'm in a perpetual state of d?j? vu but okay, I can kind of deal with it. Now recently I've gotten feeling back in my lower back and my left hip. I'll tell you what, I have sympathy for anyone who has back pain. My back hurts so bad at night that sometimes I can't sleep the matter what I do for pain medication. To top that off, my left hip feels like it's broken although we know it's not. Laying on my left side hurts really bad along with shoulder pain on the right side so laying on the right side is no fun either. Well that's all I have to complain about now. I'm just freaking miserable with this pain.

Oh, and I do have an excellent pain doctor who does my baclofen pump and everything. I'm very reluctant to put pain meds back in my baclofen pump because last time I did that they just made me dumb. She's done great work with spasms and is listened to me about all my problems. We're going to try the spinal stimulator next but I'm having a hard time getting approval for just the testing stim.

As sitting down in the commode even though that's what I've been doing since I left the rehab started to take hours and not being fully "affective". So I started doing it on my side and comes much quicker and larger. But does doing this continuingly on my side potential side affects on intestinal system more junk left behind because it's not aided by gravity

Started taking 2.5 mg Melatonin every night before bed and helping with pain!!!

It seems like it's helping my Neuropathic pain even during the day, maybe it's just because I'm sleeping better and getting a good nights rest seems like a good way of cutting down pain. Anybody else try Melatonin and any luck with it? I'm just chewing on a gummy a couple hours before bedtime, seems like I get real tired and am more ready for getting some sleep than I otherwise would be.

Been a long time, 8 years very incomplete. I just had surgery for an esophageal diverticulum related to my cervical fusion and because of complications in the healing process I am going to be fitted with a gastric feeding tube and NPO for a while.

Anyway, I am looking into how to go hiking and biking with this thing. For biking I'm mainly thinking about hydration, but for hiking I'd probably want to be able to get some nutrition as well.

I have a lightly used Superarm 2000 lift for sale, for alternate "forward" mount on wall/panel behind passenger seat as required for some vehicles like the 1989 VW Vanagon I had it in (needed to mounted forward so as not to interfere with deploying the pull out bed). I bought the lift new and sold the van after 2 years of declining use, removing the Superarm lift, which is being carefully stored at Breakaway Mobility in Boise, ID. I have a lightly used Superarm 2000 lift that was installed new for less than 2 years in a 1989 VW Vanagon. It is a rare "forward" mount - mounts to wall behind passenger seat, because mounting to the more common rear wall interfered with deploying the fold out bed in the vanagon. Colon disturbance

This part of mine has a more regular occuring twitchING as best described. 4-5 days between bathroom times is my norm. Have days mostly around the day b4 BM day where I can feel the waste just sitting in that area and it will burn & also twitch. It's twitching now & just had a recent bathroom visit.. my routine has just worked out that way and slowly, I do a lactulose drink & have to wait at least 8 hours to push it all out but plus side I don't need assistance...
Assuming best bet would be a colonoscopy & most likely any response here aye?!
Yet any ideas nurse? What could be happening there? I'm weary of it.

Hello all,

I'm looking for more information on my diagnosis and prognosis, here is the story:

21 year old, obese, couch potato. 4-6 month history of partial numbness in lower legs (worst in soles of feet) and less severely in upper legs, urinary urgency, at times constant urge to urinate despite empty bladder, sensation of weakness and numbness often worse when standing up, sitting at a desk chair often made numbness worse. Some parathesia in legs. Symptoms fluctated a lot, the only thing that was totally constant was the numbness in soles of the feet.

Family history of severe arthritis and osteo-degenerative diseases - dads spine is severely affected (still walk but on crutches and wheelchair). Mum has degenerative spinal problems but they aren't a main problem. Can't be more speific because they can't remember what's wrong with them

Naturally I go to see a neurologist. After what I imagine was a thorough examination he concludes that its all likely 'just' functional systems but says an MRI is needed to exclude inflamatory diseases. Makes sense to me.

MRI results and consult took place today. In addition the neurologist seems to be under the impression, despite my symptoms having improved a somewhat, recently that my exam clearly indicated something wrong this time (I suspect once I told him I had mental health problems at the first exam, even if he saw me in A&E after being hit by a bus he was determined it should be functional symptoms). I copy the radiologists report in full:

Cervical spine
Normal Appearance.
No central canal or foraminal stenosis
normal cervico-medullary junction and cervical cord with no suggestion of any inflammatory process.

Thoracic spine
T11/T12 prominent broad based disc herniation causing severe central canal stenosis with compression of the cauda equina and focal malacic change.
Mild mid thoracic degenerative change. No further significant central canal or foraminal stenosis.
Normal appearance of the remainder of the thoracic cord.

Lumbar spine

Constitutionally markedly narrow lumbar canal.
No central canal or foraminal stenosis at any level.
Normal conus and cauda equina

Inclusion
T11-T12 disc herniation compressing the cord with focal malacic change.

I'm happy with my understanding of what a disc herniation is. However my understanding is; when ever a doctor uses the word severe, something should be falling off or you should be dead within the week. I'm well aware that my symptoms are very mild compared to what they might be (and no pain thankfully).

So how to I have a severe disc herniation with minimal symptoms?

I'm 21 years old. Why do I have "Mild mid thoracic degenerative change" or is this related to the herniation.

My understanding of spinal anatomy is limited...to say the least, but I'm reasonably sure the cauda equina further down. Why is this affected?

I also understand that when things presss on the cauda equina it is a medical emergency requiring urgent surgery; I'm very relieved not to be needing any slicing up just yet. Why is this not an issue?

focal malacic change...stuff is getting soft in my spine. I always thought i should be perfering soft to hard in a spine. Anyway what does that mean and why are things going all mushy?

What is a constitutionally markedly narrow lumbar canal when its at home...? (I know where it is) - is this just a red herring - everyone has odd things wrong with them, it just often doesn't cause any problems?

I've been referred to a spinal surgeon for a consult. The neurologist seemed to think that an operation would be little short of insane given risk/benefit ratio (considering my minor symptoms) and I tend to agree, but also thought it important to get confirmation.

He seemed quite hesitant to hand out a prognosis and wanted that left to the surgeon. I'm also having a bladder scan to...something or other... and if so i get shiny new pills with which I am sure, delightful side effects with be included.

Can any of you lovely people shed some light on my prognosis? Will this all go away, will it stay much the same, am I going to wake up tomorrow in mind numbing agony? Not a clue?

Baring in mind I don't move much can light be shed on how I managed to herniate a disc? Is it possible my weight was involved? Would losing weight help?

Any other relevant information you can provide would be well received.

If you want more information, I'm more than happy to provide it. Unless its my pin number, I won't fall for that one, haha.

Hi, I'm potentially looking at getting a new vehicle in the summer or fall of 2016. I recently demo'd the Braun Ford Explorer conversion. While at the conversion dealership I asked the salesman if he could give me an estimate on the trade-in value of my vehicle, which is a '08 Toyota Sienna Braun conversion with passenger side powered ramp, with 123,000 miles. The inside carpeting will need to be replaced. He quoted a trade-in value of only $5k, which I thought was somewhat ridiculous given they are selling similarly aged/mileage Sienna's for $28k. If anyone has better knowledge of this, is $5k to be expected?

What could I use to patch a tiny hole in my Activeaid 101?

It is a tiny hole in one of the creases that has cracked over time. I called Activeaid to see if they made a patch but they don't and suggested I buy a new cushion for a mere $100.... That's what I paid for it 8 yrs ago, besides the rest of the chair is in excellent shape. I thought about using a rubber patch (like on a tire) but thought that might not let me slide on the seat. I would love suggestions.

I borrowed my dads laptop the other day (with Windows 10) and when I clicked on Ease of Access and then the onscreen keyboard I got a help menu. I didn't have time to see whether or not I could get it to load. Also, I use TightVNC to control other computers in the house and their website doesn't make it clear.

This one's new since Feb. & even case worker unsure if it would hold cause the extravagant hike in $$.
They're now doing care assessment by increments of the hour if you have live in 24 hour care. This goes 4 elderly/dementia/brain disorder/physical etc..
And u have to convince or break down your daily routine to where yes something is being done for care wise every single hour..
The whopper is they're shelling out $4000 + to caregivers as monthly..This is in lieu of opposed to being tossed in a nurse facility to where they're paying $6000+ monthly for us..
So we'll see what happens with it.
I'm not flaunting this in any way I know some/lots can barely scrape by melding funds life/care..just pointing it out as policy/budget/ change..

How do complete -plegics recognize injuries/problems below their site of injury?

This has always been a concern of mine, but, aside from visible physical signs, how do those of us who have any sort of complete -plegia keep an eye out for problems that fully able-bodied people are otherwise able to detect because they can feel pain or discomfort?

Example problems include things like appendicitis, or, as is the case with me right now, having just had rubber band ligation done on some hemorrhoids (again). For the latter, the possible complications that might arise (like banding too far down into the dentate line, infection, etc) are things I feel someone with sensation could notice earlier on (because of excessive pain, etc), but that a SCI patient won't notice until it's too late, due to fever, feeling like crap, etc.

Lateral rotation with LAL mattress

I have noticed that the lateral rotation component available with some LAL mattresses doesn't seem to get good reviews from writers in this column. Could you please explain why this is so? I thought that that option would be a bonus, but don't want to order one if it isn't that useful. My 18 year old son is spending time on a clinitron bed after a stage 4 sore, so we need to get a different mattress for him. He is at a SCI center, so we do have guidance from staff, but I haven't gotten much guidance on the rotation part.

Bipap machine for use with trach and/or trilogy 100 with bipap settings

Anthony is in the hospital now. He has had two bouts of pneumonia and a major surgery for duodenum bleeding ulcer all happening in December and January. Anthony's Dr. wants Ant to have access to bipap at night when he comes home. Anthony has a trach. His Dr. wrote and order to case management to have him come home with with a Trilogy 100 vent but I think he put it to Bipap settings. The company that case management sent over the order to says they cannot accommodate him because the trilogy 100 is a non invasive machine, meaning it is to be used with a mask and FDA has not approved this machine for use with a trach because there are no alarm settings. Anthony does not need a vent to breath just a little help at night.

I have called another DME and they said they actually have Bipap machines that they can set him up to use with his trach.

I want to give the Dr. information on how the order should be written for Medicare to cover this. I was told by the 2nd DME that if the Dr. is ordering a trilogy and has it set to bipap settings then Medicare would probably not cover it.

Anyway tomorrow is his last day for care and the only thing holding us up from bringing him home is getting whatever we need to get so he can have bipap at night. Has anyone with a trach had this problem and any advice on equipment and how the order should read for medicare?

Is anyone on here with a trach and uses a bipap machine at home? What is your setup? How did you get it?

Clamping some type of "grab bar" for tennis/basketball Wheelchair?

Has anyone tried or know of where I can get some type of "grab bar" to put on my tennis wheelchair (see attached pic) on my left downtube.

When I play tennis, simply grabbing the "down tube" (see arrow in picture) takes too much time. If I had some type of handle bar attachment that is a bit higher (that clamps) I would be a lot more steadier when my "non-racket" hand grabs the tube for stability while I swing. I tried using some insulation tube on the downspot, but it still has the same issue.

I've tried grabbing the left wheel itself, but as many wheelchair tennis players know, the wheelchair moves during your follow-through and isn't as steady on the ball as if grabbing the downtube.

Can someone offer some suggestion(s) of what I can use that can clamp on?

OMG! What a keyboard

I have pondered the suggestions given in my earlier thread and was able to check some of them out at BestBuy. I concluded that mechanical keys were the way to go. I ultimately selected to go with a computer keyboard made by WASD Keyboards. http://ift.tt/VyQUXz The price was hefty but well worth it because of the service I got. They custom made my keyboard for use with the typingstick keyboard layout I use. I pasted a photo below. After some discussion I decided to go with the Cherry MX brown switches. They only require 45gms of pressure for actuation and emit an audible click. My days of forcing keys down with my typing stick are gone along with tendinitis. The keys are so much easier to press. I encourage everyone to play around with a mechanical switch keyboard when you get a chance. It is something people who use a typing stick and easily tire or get sore to should definitely check out. The keyboard brings back memories of IBM electric typewriter keys. Those of you who have never typed with an IBM electric don't know how much better typing can be.

Suprapubic tube question for SCI_Nurse

Nurse,
Can you tell me whether putting a valve from a leg bag directly into the end of a suprapubic tube is dangerous? I have had a SB for years and want to wear leggings, BUT that can't really happen (and be cute) while having a leg bag. If I cut the valve out of a Bard bag and drained it at least once an hour or so, is that safe? Probably not recommended, but it wouldn't be a regular thing, only if I wear leggings.
