Scab after Stage 4 ulcer questions

Good evening everyone. To make a long story short, I developed a stage 4 pressure ucler with osteo on my left ischial. If interested I can explain how I got to stage 4 due to a VA hospital incompetence. I was admitted to another VA hospital, one I went to when first injured and SCI hospital, and stayed there for several months until it was healed enough for me to go home. During the last days there a small scab developed over the pin size hole, so small that the long stick q-tips wouldn't fit in it. I was discharged in early July of this year, and since then the scab has gotten a little bit bigger and sometimes it bleeds. I wear a band aid over it during the day while I'm up but take it off at night when sleeping fyi.

My questions are:

1. Is it normal for a scab a bit more bigger than the wound area?
2. Does bigger scab mean the wound area is getting bigger?
3. Is bleeding through the scab ok? Sometimes will be like a little drop a blood or sometimes looks like 1/2 the band aid area is stained.
4. Should I be putting the lotion(http://ift.tt/1RQi1K1) they gave on the scab? I've been told also not to put anything on it cause it could build up and cause an infection if it got inside the scab.
